Job Summary
Provide integrated counseling and case management services to stabilize lives and promote housing stability for formerly homeless individuals with chronic medical conditions. Collaborate on client-centered recovery plans while utilizing a harm reduction approach to support mental health and substance use needs. Conduct regular apartment visits, community outings, and escort residents to appointments, developing safety plans and managing crises. Make referrals to care coordination agencies, health providers, and employment services, and develop inclusive groups and programs. Maintain confidential records and participate in clinical meetings, supervision, and evidence-based training. Work within a supportive housing environment, collaborating with the Leasing and Compliance team to ensure stability.
Required Qualifications
- MSW
- Ability to work independently as well as a member of an interdisciplinary team
- Highly organized and able to manage a diverse caseload of clients with varying needs
- Able to work well under pressure, including in crisis situations, and flexible with work schedule when necessary
- Experience with ETO or other electronic record-keeping systems
- One year of experience working with adults and/or families impacted by poverty, chronic homelessness, mental illness, substance use, or medical issues
Desired Qualifications
- LMSW
- Bilingual Spanish/English
- Knowledge of HIV/AIDS, mental health, substance use, or chronically homeless populations
- Experience working with evidence-based models, specifically Motivational Interviewing
