Develop comprehensive social assessments and individualized care plans with measurable goals and interventions. Participate in pre-admission and admission processes, orienting residents and families to the facility. Serve as a resource for residents and families during adjustment periods, crises, or special circumstances while documenting behavioral and psychosocial status accurately. Provide individual and group support services, establish volunteer programs, and collaborate with the interdisciplinary team to promote resident-centered care. Ensure compliance with federal, state, and local regulations governing social services in long-term care.
