Job Summary

Meet with residents and families to review policies on advanced directives and rights, then conduct social services evaluations and document identified needs. Assist with admissions paperwork, tours, and referrals while maintaining community resource lists and pursuing protective services when necessary. Provide/arrange social work services based on resident/family needs, coordinate family meetings, and facilitate RAI processes to update care plans. Track PASARR and OBRA screenings, implement treatment interventions, and ensure all services comply with state, federal, and accreditation guidelines. Maintain confidentiality and demonstrate respect for resident dignity while participating in quality improvement activities and continuing education.

Required Qualifications

  • Minimum of bachelor's degree in Social Work
  • One year supervised social work experience working directly with individuals in a long-term or sub-acute care rehabilitation facility
  • Currently licensed as Social Worker in State of Michigan
  • Knowledge of regulatory standards and compliance requirements
  • Working knowledge of personal computer and software applications used in job functions (word processing, graphics, databases, spreadsheets, etc.)
  • Strong organizational, analytical, oral communication, and written communication skills
  • Working knowledge and ability to apply professional standards of social work practice in job situations
  • Current knowledge of state and federal laws and regulations that apply to the practice of social work in long-term care
  • Ability to read, analyze, and interpret professional journals, technical procedures, or governmental regulations
  • Ability to communicate orally and through written reports and other documents relating to patient, staff, and others
  • Ability to effectively present information and respond to questions from groups of executives, managers, clients, customers, and the public
  • Ability to define and solve problems, collect data, establish facts, and draw valid conclusions
  • Ability to interpret an extensive variety of technical instructions in statistical or diagram form and deal with several abstract and concrete variables
  • Sufficient mobility and strength to move freely through the building, to assure resident safety at all times, and to assist, transfer, or otherwise move residents of facility out of danger in case of emergency
  • Ability to locate the nearest exits, and understand and respond to written or oral instructions in case of emergency
  • Ability to comply with facility policies and procedures for workplace safety, including infection control procedures, application of universal precautions for blood borne pathogens, use of personal protective equipment, and handling of hazardous materials
  • Ability to prioritize tasks/responsibilities and complete duties/projects within the allotted time
  • Able to carry out the essential functions of this job (with or without reasonable accommodation), without posing risk of substantial harm to the health and safety of self and others
  • Ensure that the quality and appropriateness of resident care meets or exceeds company and industry standards, and that all services are in compliance with state, federal, legal, regulatory, accreditation, and reimbursement guidelines
  • Attend in-service and educational programs; attend continuing education required for maintenance certification
