Job Summary
Conduct comprehensive bio-psychosocial assessments at enrollment and 180-day intervals, utilizing cognitive behavioral techniques and motivational interviewing for counseling. Develop individual treatment plans and coordinate services with the Interdisciplinary Team, managing housing, financial, and safety issues while assessing crisis risks. Direct case management, facilitate discharge planning, and advocate for participants in resolving concerns. Document all activities accurately in the electronic health record and evaluate progress toward care objectives. Requires a Master's degree in Social Work, Michigan licensure, and one year of clinical experience.
Required Qualifications
- Master's Social Worker from an accredited school of social work
- Licensed to practice in the State of Michigan
- One year experience working as a clinical social worker
- Overall knowledge of case management skills
- Knowledge of and compliance with applicable local, state, and federal laws and regulations
- Ability to learn and use electronic health record system
- Proficient in Microsoft Word, Excel, Outlook, PowerPoint
- Positive, flexible, and solution-focused attitude
- Ability to collaborate effectively in a team setting
