Job Summary
Provide social work services to hospital patients and families, including discharge planning, psychosocial assessment, and advocacy; serve as liaison between hospital and community resources; supervise social work interns; coordinate care and education for patients and families, and support transition in level of care; requires Minnesota social work licensure (LICSW preferred) and prior health-care experience; day shift 7:30 AM–4:00 PM, Monday–Friday.
Required Qualifications
- Must hold a current,unrestrictedMinnesota State licensure for Social Workers
- Licensed Independent Clinical Social Worker (LICSW) licensure is preferred
- Prior experience in a health care setting required
- Acute care setting is preferred
