Social Worker (Residential Living)

Coordinate admissions and welcome new Residential Living residents while serving as an advocate and trusted resource for residents and their families. Assist with transitions, discharges, and moves throughout the community, collaborating with the Wellness Team, Personal Care, Skilled Nursing, Marketing, and Life Enrichment departments. Resolve resident concerns, promote independence and satisfaction, and plan wellness programs, educational opportunities, support groups, and resident events. Communicate with outside providers to coordinate services, monitor well-being, document incidents, and ensure compliance with community policies. This role supports a warm, engaging, person-centered environment at Peter Becker Community, a faith-based nonprofit Continuing Care Retirement Community recognized as one of Pennsylvania's Top 3 Nursing Homes.
