Job Summary
The Social Worker will provide social service support to tenants and POP Management staff, coordinating care and services across social service teams in senior properties. Responsibilities include planning, coordinating, and facilitating social, wellness, and recovery groups; documenting activities; coordinating targeted high-need cases; conducting field visits and meetings with tenants, families, and social service coordinators; overseeing screening, admission, and discharge processes; supervising and training staff at the BA level; ensuring chart audit readiness and HIPAA/compliance; liaising with multiple departments and agencies to address health, housing, and wellness needs; participating in staff development and CQI initiatives; and responding to crises in collaboration with leadership. Strong organizational, verbal, and written communication skills are required, with bilingual abilities preferred. Must be able to work flexible hours and travel within the five boroughs.
Required Qualifications
- Master’s in Social Work, Mental Health Counseling, Human Services, Psychology or related field
