Job Summary
Provide comprehensive social work services to geriatric residents in the Skilled Nursing Facility, focusing on short-term rehabilitation and long-term care populations. Deliver psychosocial evaluations addressing medical social needs, emotional factors, and adjustment to care. Facilitate discharge planning by identifying psychosocial, environmental, and financial barriers, collaborating with RNs and Case Managers to implement optimal transition strategies. Assist with financial needs through charity funds and disability applications while documenting continuous progress by end of business day. Maintain compliance with Joint Commission regulations and participate in community engagement activities. Travel is sometimes required.
Required Qualifications
- Bachelor's degree in Social Work, Psychology, Mental Health Counseling, or related field
- Minimum two years post-graduate experience as a social worker in a similar type of healthcare setting
- Valid license to practice as a social worker
- Ability to provide psychosocial evaluation to meet the patient's medically related social needs
- Ability to facilitate discharge planning to meet the patient's needs for continued care
- Ability to collaborate with the healthcare team (including RNs and Case Managers) to establish and implement an optimal discharge plan
- Ability to document appropriately and timely to reflect continuous progress in the plan of care
- Ability to assess and facilitate financial needs by utilizing charity funds, assisting with applications, and collaborating with the Business Office and outside agencies
- Knowledge of rules, regulations, policies, and laws, including Joint Commission, that impact the Social Work field
- Ability to travel
- Ability to participate in community and employee engagement activities
