Job Summary
Social Worker at Regency Manor provides direct psychosocial support to residents, develops and carries out plans to maximize residents’ potential, educates residents and families about post-acute options and advance directives, assists with discharge planning, and coordinates with the interdisciplinary team. Core duties include completing and documenting social work and psychosocial assessments within 3 days of admission, ensuring accurate completion of 3877/78 forms, scheduling and notifying the IDT for 48-hour and quarterly/annual Resident Care Conferences, completing MDS sections and CAAs, maintaining and updating care plans (psychosocial, discharge planning, behavioral, cognitive loss, mood, vision/hearing, podiatry, dental), monitoring behavior logs and high-risk residents, coordinating with psychiatric providers and ancillary services (dental, ophthalmology, audiology), educating residents and families about advanced directives and code status within 48 hours of admission, ensuring daily visits and assessments for residents involved in untoward events, documenting all interactions in EMR, contributing to ongoing staff training, participating in Medicare meetings and QAPI, and arranging DME and home care at discharge. Requires a Bachelor’s degree and at least 1 year of supervised social work experience in a healthcare setting. Location: Utica, MI (Regency Manor, Utica, MI), US.
Required Qualifications
- Bachelor's degree in social work – or – a Bachelor's degree in a human services field including but not limited to sociology, special education, rehabilitation counseling, gerontology and psychology required
- 1 year of supervised social work experience in a health care setting working directly with individuals required
