Job Summary
The Social Worker/PRSC coordinates and mentors each resident’s psychiatric rehabilitation services program for a specific caseload, integrating evaluation, planning, and monitoring of services to meet emotional and social needs. Responsibilities include establishing therapeutic relationships, orienting residents, reviewing treatment plans, facilitating active participation in plans, coordinating rehabilitation services, and monitoring self-directed care and overall adherence to the treatment plan. Duties span administrative tasks (policies, reporting, transportation arrangements, staff consultation, charting), psychiatric rehabilitation functions (assessments, care-plan conferences, 1:1/group counseling, skill development, documentation), and collaboration with the interdisciplinary team to address social, familial, and environmental factors affecting resident outcomes. Requires a bachelor’s degree in human services and at least one year of supervised mental health experience; full-time role based in Westmont, IL.
Required Qualifications
- Bachelor’s degree in a human services field (including but not limited to sociology, special education, rehabilitation counseling or psychology)
- minimum of one year of supervised experience in mental health or human services
