Job Summary
Social Worker PRN providing case management and program coordination for rehabilitation patients to ensure clinical and financial outcomes. Manages treatment plans, coordinates length of stay within CMS guidelines, and connects patients with community resources, including vocational rehabilitation. Works with diverse disabilities (e.g., spinal cord injury, brain injury, CVA, amputation, orthopedic conditions, arthritis) and collaborates with physicians to achieve timely, cost-effective, individualized service delivery. Tulsa, OK location with in-person duties.
Required Qualifications
- Current state licensure as a Registered Nurse or Licensed Social Worker
