Job Summary
Hospice Social Worker (PRN) handles psychosocial assessments, counseling, and care planning for patients with terminal illness and their families. Conducts social evaluations, educates interdisciplinary team on cultural needs, maintains clinical records, assists with advanced directives, provides information and referrals to community resources, liaises between patients/families and agencies, participates in interdisciplinary meetings, and supports funeral planning and decision-making. Requires a Master’s in Social Work, LSW licensure, and willingness to travel across Bristol Hospice service area; travel and flexible hours expected; prior hospice experience desirable.
Required Qualifications
- Must have a master’s degree in social work from an accredited university.
- Must be a Licensed Social Worker (LSW)
- Must have minimum of two (2) years of documented supervised experience in health care, hospice experience preferred
- Must understand hospice philosophy, and issues of death/dying.
- Certified Hospice and Palliative Social Worker is desirable
- Must be flexible in work hours and have the ability to travel throughout the assigned Bristol Hospice service area
- Must demonstrate a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs
