Job Summary
Assess psychosocial status for RN Case Managers and interdisciplinary groups, then evaluate medical needs, home situations, and community resources to shape care plans. Provide individualized social work plans and deliver empathetic counseling to patients and families while collaborating with the Bereavement Coordinator. Address psychosocial stresses during Monday through Friday shifts from 8 am to 4:30 pm, with optional weekend coverage available for Greenwood and surrounding counties.
Required Qualifications
- Master of Social Work from an accredited school of social work
- Licensed as a Social Worker from the State Board of Social Work
- Current Driver's License, vehicle insurance, and access to a dependable vehicle
- Minimum one (1) year experience as a social worker in a healthcare setting
- Current unencumbered license as a Social Worker from the State Board of Social Work
Desired Qualifications
- Minimum one (1) year experience as a social worker in a hospice setting
- long term care or medical surgical/acute care setting
