Job Summary
Contract Social Worker to support the Chronic Care Management (CCM) program by providing psychosocial assessments, care coordination, and resource navigation for patients with chronic and complex conditions. Collaborates with interdisciplinary care teams to develop and execute individualized care plans, provides short-term supportive counseling, identifies and facilitates access to housing, transportation, food assistance, behavioral health, and financial services, and supports adherence to care plans and medications. Maintains compliant documentation in EHR/ CCM platforms, participates in case discussions, and ensures activities align with HIPAA and CMS CCM guidelines. Requires strong understanding of social determinants of health, excellent communication, ability to manage caseloads independently, and bilingual English/Spanish preferred. Active Florida Social Work License required; MSW strongly preferred; 2–3 years of experience in care management or medical social work; travel and telehealth familiarity may be beneficial.
Required Qualifications
- Active Florida Social Work License
- Bachelor’s or Master’s degree in Social Work (MSW strongly preferred)
- 2–3 years of experience in care management, medical social work, or chronic disease management
- LCSW preferred
