Job Summary
Licensed Social Worker with MA licensure (LCSW/LICSW) sought to assess the psychosocial status of patients, conduct social evaluations, and communicate findings to the interdisciplinary care team. Responsibilities include counseling patients and families, providing crisis intervention, educating on cultural needs, maintaining clinical records, assisting with advanced directives, offering information and referrals, and participating in discharge planning and care-plan development within a long-term care setting. Strong verbal/written communication and organizational skills required; position supports collaboration with public/private agencies and caregivers to support patient care.
Required Qualifications
- Minimum of one (1) year experience in health care
- Current Massachusetts licensure: LCSW or LICSW
- Bachelor’s degree preferred
Desired Qualifications
- Master's degree preferred
- Experience in a long term care setting preferred
