Job Summary
The Social Worker will integrate with the healthcare team at PHMC Health Network, performing client assessments focusing on Social Determinants of Health and providing connections to social service resources. Responsibilities include assessing client needs, facilitating communication between clients and providers, and assisting clients with health insurance and public benefits. Must demonstrate knowledge of community resources, possess strong clinical judgment and critical thinking skills, and have experience with diverse populations and low-income individuals. Proficiency in electronic health records and documentation is essential.
Required Qualifications
- Masters’ Degree in Social Work from an accredited school strongly preferred
- Bachelor’s required
- Minimum of 3 years’ experience in case management and social services
Desired Qualifications
- Experience with crisis intervention and harm reduction
- Experience working with those experiencing homelessness
- Experience working with families and individuals with low or no income
- Experience working as a member of an interdisciplinary team
- Comfort with evidenced-based interventions such as motivational interviewing
- Comfort working with diverse populations
