Job Summary
Deliver social services including psychosocial evaluations, treatment, advocacy, teaching, and discharge planning assistance. Interview patients and significant others to assess social and family situations, then communicate findings to physicians and the care team. Make recommendations for additional services, arrange community resources, and facilitate interdisciplinary rounds to ensure patients understand medical recommendations. Ensure documentation in the medical record is clear and complete while maintaining HIPAA compliance. Intervene in cases of abuse or neglect and make referrals to resolve patient issues effectively. Participate in departmental studies and projects as assigned. This role supports patients across various cultural and religious backgrounds, requiring empathy and compassionate care. The position requires a Bachelor's Degree in Social Work and knowledge of CMS, Medicare/Medi-Cal, and Joint Commission requirements.
Required Qualifications
- Bachelor's Degree in Social Work (BSW) or related degree in human services
- Computer Proficiency and Literacy
- Knowledge of CMS, Medicare/Medi-Cal
- Familiarity of Joint Commission, CMS, CDPH requirements
- Excellent written and verbal communication skills in English
- Ability to establish and maintain effective working relationships across the organization
- Ability to facilitate and lead interdisciplinary rounds
