Job Summary
Deliver social services including psychosocial evaluation, treatment, advocacy, teaching, referrals, and discharge planning assistance. Interview patients and families to assess social conditions, communicate findings to physicians and staff, and facilitate interdisciplinary rounds. Make recommendations for additional services and arrange community resources to support complex discharges and financial needs. Ensure medical documentation is clear and complete while maintaining HIPAA compliance. Empathize with patients and support systems across diverse cultural and religious backgrounds. Participate in departmental studies and projects as assigned by the Social Service Department.
Required Qualifications
- Bachelor's Degree in Social Work (BSW) or related degree in human services
- Computer Proficiency and Literacy
- Knowledge of CMS, Medicare/Medi-Cal
- Familiarity of Joint Commission, CMS, CDPH requirements
- Excellent written and verbal communication skills in English
- Ability to establish and maintain effective working relationships across the organization
- Ability to facilitate and lead interdisciplinary rounds
Desired Qualifications
- One year of healthcare experience
- Bilingual skills to communicate effectively with patients and families
