Social Worker - Partial Hospitalization

Provides individual and/or group social work services to patients and/or their families in various health care settings; utilizes psychosocial assessment skills to deliver social work services. Responsibilities include direct service to patients and families using a variety of social work modalities to maximize functioning; identify patients in need of assessment and intervention via high risk criteria or referrals; advocate to ensure access to health care; contribute assessment findings to interdisciplinary teams; provide psychoeducation, counseling, and crisis intervention; formulate and implement safe, timely treatment and discharge plans aligned with site expectations; coordinate with post-acute providers and payers; apply knowledge of community resources and entitlement programs; maintain professional documentation; represent department in committees; potentially supervise students and conduct evaluations of field placement performance; participate in program management discussions on training and placement; perform related duties as essential functions under ADA.
