Social Worker - Part Time

Plans and participates in the social service program for facility residents, including coordinating with community facilities and agencies to secure economic, psychological, and medical assistance; maintains caseload and completes assessments; initiates and updates care plans; coordinates discharge planning with residents, responsible parties, and interdisciplinary teams; assists residents with applications to Home Health Care, Department of Social Service, Meals on Wheels, and similar services upon discharge; participates in in-service education and supports the Director of Social Service; contributes to community planning related to the facility's interests; stays current on legislation, trends, resources, and facilities in the health field; maintains confidentiality and completes required documentation in resident charts; participates in the MDS+ process and performs other duties as directed.
