Job Summary
Assess psychosocial status for RN Case Managers and interdisciplinary groups while evaluating medical needs, home situations, and community resources. Implement personalized social work plans, collaborate with the Bereavement Coordinator, and shape care plans through interdisciplinary meetings. Provide empathetic counseling to patients and families to support emotional well-being and coping capacity. Address psychosocial stresses and advocate for individualized care within Beaufort, Jasper, Hampton, and Colleton County. This role supports Crescent Hospice's mission to bring dignity and human connection to end-of-life care.
Required Qualifications
- Master of Social Work from an accredited school of social work
- Licensed as a Social Worker from the State Board of Social Work
- Current Driver's License, vehicle insurance, and access to a dependable vehicle
- Minimum one (1) year experience as a social worker in a healthcare setting
- Current unencumbered license as a Social Worker from the State Board of Social Work
Desired Qualifications
- Minimum one (1) year experience as a social worker in a hospice setting
- long term care or medical surgical/acute care setting
