Social Worker

Conduct face-to-face resident interviews and assessments of cognitive, mood, and behavioral status alongside psychosocial factors. Coordinate, supervise, and facilitate the delivery of social work services to all residents, family members, and significant others in accordance with state and federal regulations. Participate in discharge planning, clinical evaluations for potential admissions, and interdisciplinary team meetings to access community-based services. Direct medically related social services and serve as a treatment team liaison with contracted mental health practitioners. Actively develop and implement new behavioral and psycho-social service programs. Available to respond to clinical emergencies, regulatory agency visits, and surveys outside of normal work hours.
