Job Summary
Provide psychosocial evaluations to assess social, emotional, and financial factors impacting patient care and discharge planning. Participate in interdisciplinary conferences, facilitate discharge coordination with case managers, and document continuous progress in patient plans of care. Respond to crisis interventions for trauma, abuse, or neglect, and assist families with financial applications and charity funds. Maintain 24-hour on-call accessibility and complete daily status documentation for acute patients. This role supports patients receiving or recently received hospital-based care at Guthrie's 197-bed acute care facility in Binghamton. Candidates must have a minimum of two years of post-graduate social work experience in a healthcare setting and successfully complete a six-month orientation.
Required Qualifications
- Minimum two years post graduate experience as a social worker in a similar type of health care setting
- Successful completing a 6 month hospital, based orientation process
- LMSW, LSW, or LCSW license for staff providing care to patients in the state of New York
