Job Summary
Licensed social work professional to provide individual, family, and group services to youth birth-21 and their families under the CFTSS program. Responsibilities include conducting evaluations and assessments, developing and implementing treatment plans, and performing reviews to monitor progress; complete and maintain documentation in the Electronic Health Record system; maintain client confidentiality; collaborate with staff and outside service providers; provide supervision to other members of the CFTSS team; attend supervision and staff meetings. Education requirements include a Master’s Degree in Social Work, Psychology or related field and a license/permit such as LMSW, LCSW, LMHC, LMFT, LCAT, Licensed Psychologist, or similar; a valid Driver’s License with maintained insurance is required.
