Job Summary
Social Worker position at OneWorld Community Health Centers, Inc. (full-time) on the main Livestock Exchange Campus in Omaha, NE. Provides a broad range of psychosocial services within an integrated health care team, including assessments, program development, case management, patient advocacy, outreach, and supervision of practicum students. Responsibilities include applying social work principles to client and family needs, connecting patients with community resources, assisting with transportation, housing, and other social determinants of health, and maintaining records. Requires a CSWE-accredited bachelor’s degree in social work; CSW is preferred with certification due within 90 days if not already certified. English/Spanish proficiency, willingness to perform home visits (MVR required) and hold a valid driver’s license. Prior experience with domestic violence, CPS, or APS is a plus; strong teamwork, organizational, and communication skills are essential.
Required Qualifications
- Bachelor’s Degree in Social Work (CSWE-accredited)
- Certified Social Worker (CSW) by Nebraska State Law preferred upon hiring; certification required within 90 days if not certified at hire
- Excellent verbal and written communication skills; English/Spanish proficiency
- Willingness to complete home visits (MVR required) and valid driver’s license
- Experience: two years social work experience in healthcare or social services preferred
- Familiarity with social service resources in the community
- Ability to work in a team and manage casework and advocacy responsibilities
