Job Summary

Assess patients and families for treatment goals, psychosocial factors, and discharge barriers while coordinating care with agencies, payors, and post-acute resources. Provide discharge planning, loss and grief support, crisis intervention, and referrals for mental health and substance use disorders. Bridge communication between the care team and families to address cultural or financial barriers impacting care progression. Intervene in cases of child abuse, domestic violence, or assault as required. Maintain knowledge of government entitlement programs and facilitate advance directives or guardianships during emergent status changes.

Required Qualifications

  • Master of Social Work Degree from a program accredited by the Council on Social Work Education
  • one year of full-time social work experience in a health care setting
  • experience in a hospital setting providing direct service to patients and their support systems
  • excellent assessment and clinical intervention skills
  • experience in discharge planning
  • experience connecting clients with community services
  • strong organizational skills
  • customer service skills
  • Agency Affiliated Counselor credential
  • Licensed Social Work Associate -Advanced credential
  • Licensed Social Work Associate - Independent Clinical credential
  • Licensed Advance Social Worker credential
  • Licensed Independent Clinical Social Worker credential
