Job Summary
Social Worker required to join an Older Adults Team in Manchester. Deliver high-quality, person-centred social work support to older adults with complex needs, promoting independence, safeguarding wellbeing, and ensuring compliance with Care Act 2014 and related legislation. Responsibilities include undertakING holistic assessments of need, risk and capacity; developing and reviewing personalised care and support plans; conducting Mental Capacity Act assessments and Best Interests decisions; identifying safeguarding concerns; advocating for the rights, dignity, and autonomy of older people; collaborating with health professionals, housing teams, voluntary organisations, and care providers; maintaining accurate case records; ensuring statutory duties and local policy compliance; acting as an advocate in decision-making. The role requires Social Work England registration, UK driving licence, post-qualification experience, and a degree in Social Work.
