Job Summary
Social Worker (MSW) provides psychosocial assessment, discharge planning, resource coordination, and patient/family support to facilitate safe, timely transitions in acute care. Works with an interdisciplinary team to identify barriers to discharge, develop individualized discharge plans, coordinate post-acute services (home health, DME, transportation, community resources), and educate patients and families on available resources and benefits. Facilitates safe transitions to SNF/IRF/LTACH or home, supports advanced care planning (non-clinical), advocates for patient rights and culturally appropriate care, and participates in daily rounds and care coordination meetings. Does not provide psychotherapy or licensed clinical interventions. Requires a Master's in Social Work and active licensure as a Licensed Master Social Worker (Tennessee) (or licensure within 12 months). Preferred experience in acute care and knowledge of discharge planning and healthcare systems. Strong interpersonal, organizational, and communication skills; ability to link patients to community resources and collaborate with physicians and care teams.
Required Qualifications
- Master's Degree in Social Work
- Licensed Master Social Worker Tennessee (or licensed within 12 months)
- Experience in acute care, healthcare, social services or community resource coordination preferred
- Knowledge of discharge planning, community resources, and healthcare systems strongly preferred
- Strong interpersonal and communication skills
- Proficiency with basic computer applications and electronic medical record systems
