Job Summary
The Social Worker at Caris Healthcare supports the hospice interdisciplinary team by completing comprehensive patient assessments and providing psychosocial and emotional support to patients and caregivers at the end of life. Responsibilities include identifying care planning goals, participating in Interdisciplinary Team meetings, providing funeral planning education, conducting visits in residential settings, assisting with bereavement support, and participating in on-call rotations for after-hours needs. A Master's or Bachelor's degree in Social Work is preferred or required based on state licensure, along with one year of experience in a healthcare setting.

- MSW or BSW (as per state requirements)
- One (1) year of social work experience in a health care setting
