Job Summary
Conduct psychosocial and bereavement assessments for patients and families within five days of hospice admission, documenting interventions and outcomes in the Electronic Medical Record. Collaborate with the Interdisciplinary Team to formulate and update Plans of Care, discuss these plans with admitting nurses, and facilitate discharge transfers. Assist families with insurance coverage, Medicaid applications, funeral arrangements, and community resource identification while communicating patient condition changes to the attending physician. Participate in IDT meetings, quality improvement processes, and staff meetings, and deliver or supervise social services including after-hours on-call visits for crises or deaths.

- Social Work Licensure required
- LMSW preferred
- Min. LBSW required
- Baccalaureate degree in Social Work
- Ongoing supervision and oversight by an MSW
- One (1) year of social work experience in a health care setting
- Must be willing and available in an on-call rotation to make after hours visits (i.e. on weekday evenings or on the weekend) when needed for a patient's death, funeral, or admission, etc. or psychosocial crisis

- South Carolina = Social Work Licensure Required; LMSW required
- Georgia = Social Work Licensure required; LMSW preferred, Min. LBSW required
- If minimum requirement is a Baccalaureate degree in Social Work. This is only available as long as Caris is able to arrange ongoing supervision and oversight by an MSW
