Job Summary
Provide psychosocial support and care coordination for people with chronic and complex health conditions in the community; conduct holistic psychosocial and risk assessments; develop and implement individualised care plans in partnership with clients, families and multidisciplinary teams; provide short to medium-term therapeutic support, including counselling for anxiety, depression, grief, stress, family violence and adjustment-related concerns; support goal setting and self-management, strengthening people’s capacity to navigate health and community services; assist with care navigation, including support with housing, income support, NDIS and other service systems; coordinate access to supplementary services funding to reduce financial barriers and promote equitable access to care; participate in case conferencing and multidisciplinary meetings to enhance continuity and quality of care; maintain accurate clinical documentation and deliver services in line with professional, ethical and legislative standards; contribute to quality improvement initiatives and positive team culture.
Required Qualifications
- Bachelor’s degree in Social Work
- Minimum two years’ experience working as a qualified Social Worker
- Current driver’s licence and willingness to travel for community and home-based work
- Criminal History Check
- Employee Working with Children Check
- National Worker Screening Check
