Job Summary
Weekends-only social work role collaborating with an interdisciplinary team to assist patients and families with complex psychosocial needs. Responsibilities include psychosocial assessments, discharge planning, addressing abuse/neglect, adoption, substance use disorders, end-of-life decisions, resource referrals, bereavement support, adjustment counseling, facilitating guardianships, and behavioral health concerns (including psychiatric transfers). Conducts assessments with patients and/or families, documents plans in the electronic medical record, communicates with alternate sites and community agencies, and stays current on federal and state regulations relevant to the role. Provides consultation and resources to healthcare team and participates in stakeholder committees or projects as requested.
Required Qualifications
- Master's Degree in Social Work (MSW)
