Social Worker, MSW

Social Worker, MSW (full-time) provides social services to patients and their families, including counseling and support, and establishes a care plan to foster personal worth, spiritual well-being and human dignity. Core duties include collaborating on patient discharge plans, reassessing plans of care, conducting patient assessments (illness, mental status, family dynamics), documenting data per laws/regulations, linking patients to community resources, acting as patient advocate and liaison to physicians/insurance/agency partners, guiding patients and families to medical recommendations, and providing clinical supervision to junior social workers while adhering to institutional policies.
