Job Summary
The Social Worker – MSW provides clinical social work services to patients and families across the healthcare continuum, conducting age-appropriate psychosocial assessments, offering counseling and resource education, coordinating care and discharge planning, and facilitating referrals to internal and external services. Works with an interdisciplinary team to address barriers to care, supports transitions to post-acute services, and participates in quality improvement and documentation in the medical record; position requires MSW, LMSW licensure, and BCLS with 2–4 years of healthcare social work experience.
Required Qualifications
- Master's Degree in Social Work (MSW) from a program accredited by CSWE
- LMSW - Licensed Medical Social Worker - State Licensure required
- BCLS - Basic Life Support required
- 2-4 years of clinical social work experience in healthcare setting preferred
