Job Summary
Provide social work services to the Stroke & Neurological Rehab Care department across the hospital. Responsibilities include assessment of patients, crisis intervention, individual and family treatment, comprehensive discharge planning, and community liaison. The role may rotate across units as needed. Requires an MSW and current OCSWSSW registration; hospital experience and experience with continuing care, patient/caregiver counselling, and LTC placement are preferred. Working evenings and weekends is required. Temporary position up to 5 months. Salary ranges from $44.67 to $57.90 per hour.
Required Qualifications
- Master of Social Work degree
- Current registration with OCSWSSW
- Previous related hospital experience preferred
- Previous experience with continuing care patients preferred
- Patient and caregiver counselling and LTC placement experience preferred
- Must be available evenings and weekends
