Social Worker, MSW, PRN, Days

Promote patient and family wellness and manage comprehensive transitional care plans for patients with complex and psychosocial needs in Inpatient and Emergency Department environments. Support improved care outcomes and access to appropriate hospital and community resources. Requires a Master's degree from an accredited social work program and two years of experience in an acute or post-acute setting. LMSW licensure in Georgia is preferred. This role is part of the Social Work team at Piedmont Rockdale Hospital.
