Job Summary
Assess psychosocial needs of participants and families based on emotional, environmental, financial, psychological, cognitive, social, cultural, and health factors. Establish therapeutic relationships, provide short-term individual counseling, and collaborate with the interdisciplinary team to develop, evaluate, and revise comprehensive care plans. Initiate referrals to community resources, assist with Medicaid applications, and implement care goals while documenting interventions in a risk-neutral fashion. Participate in the Counselor On-Call rotation and monitor reports including QA, satisfaction surveys, and budget data. Requires a Master's in Social Work, one year of experience with older adults, and a valid Colorado license. Schedule is Monday-Friday, 8-5 in Greeley, CO.
Required Qualifications
- Master's degree in Social Work
- One year experience working with older adults
- knowledge and skills with regard to health care coordination
- knowledge and skills with regard to pain/symptom management
- knowledge and skills with regard to crisis intervention
- knowledge and skills with regard to counseling
- Valid Colorado Driver's License
- proof of Auto Insurance in accordance with state statute
- Valid BLS Certification
- Ability to lift/push/pull minimum of 30 lbs
Desired Qualifications
- Three to five years of professional social work experience in health care or mental health
