Job Summary
Social Worker for the PACE program provides psychosocial assessment, care planning, and coordination of care; conducts interventions including individual and group modalities, family and collateral contacts, education, and discharge planning; collaborates with an Interdisciplinary Team to optimize health status and quality of life for PACE participants; requires MSW and at least 2 years of relevant experience, with bilingual ability strongly preferred.
Required Qualifications
- Master’s Degree in Social Work (MSW) from an accredited university
- Minimum of 2 years of experience in case management, social advocacy, and/or mental health with geriatric population
- Field work/internship may substitute 1 year of experience
- BLS certification or higher per AHA/Red Cross guidelines
