Job Summary
Administer assessment tools and gather data to evaluate social, emotional, and cognitive status for patients in primary care, acute, and chronic disease settings. Interview families to determine social work needs and coordinate community resources to provide comprehensive care. Conduct initial and ongoing assessments involving participants and staff while ensuring accurate documentation for all treatment plans and evaluations. Requires Master's Degree in Social Work with two years of behavioral health experience.

- LICSW
- Experience working in a hospital/acute situation or equivalent social service agency, experience relative to the medical social work function
- Minimum two (2) years of experience in a behavioral health setting
- BLS - Basic Life Support
- First Aid
