Job Summary
Provides psychosocial assessment, counseling, crisis intervention, and information/referral services on a consultation basis. Works collaboratively with the management team to ensure the efficient utilization of resources for positive patient outcomes. Responsibilities include employing counseling skills to help patients/families understand and follow care recommendations, staying current with community resources for social, emotional, and financial assistance, intervening in crisis situations using crisis intervention theory, identifying and recording clinical and financial barriers to smooth transitions within the healthcare network, participating in audits to ensure adherence to laws and guidelines, and interviewing patients to complete initial psychosocial evaluations while reporting suspected cases of abuse. Requires a Masters Degree in Social Work and at least 1 year of relevant experience; strong emotional intelligence, empathy, NASW Code of Ethics knowledge, and HIPAA awareness are emphasized.
Required Qualifications
- Masters Degree in Social Work
- 1 year experience working as a social worker or applicable mental health experience
- Ability to work effectively with people from a broad range of backgrounds
- Knowledge of patient rights and laws relative to those rights, such as HIPAA
- Knowledge of psychological principles and theories
- Skill in using logic and reasoning to identify strengths and weaknesses of alternative solutions, conclusions, or approaches to problems
- LSW - Licensed Social Worker - State of Pennsylvania (preferred)
