Job Summary
Conduct psychosocial assessments within five days of hospice admission or 30 days for dialysis initiation, including 90-day and annual reassessments for unstable patients. Provide counseling on death, dying, anticipatory grief, and adjustment to chronic illness, while evaluating cognitive status, mental health history, and financial capabilities. Navigate community resources, legal issues, and housing needs to support patients and families in end-of-life and kidney failure care. Participate in interdisciplinary group and team conferences to develop care plans, assist in discharge planning, and administer quality-of-life tools. Respond to crisis situations, provide education on treatment options and patient rights, and document all interventions in compliance with regulatory requirements.
Required Qualifications
- Master's degree in Social Work (MSW) required for dialysis services
- Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education
- baccalaureate degree in social work from an institution accredited by the Council on Social Work Education
- baccalaureate degree in psychology, sociology, or other field related to social work
- 1 year of social work experience in a healthcare setting
- baccalaureate degree from a school of social work accredited by the Council on Social Work Education
- employed by the hospice before December 2, 2008
- Knowledge of hospice and dialysis psychosocial needs, community resources, and regulatory requirements
- Strong organizational skills and ability to work independently
- Effective problem-solving, communication, and decision-making abilities
- Competent computer and documentation skills

Desired Qualifications
- Counseling experience in death and dying or orientation to the hospice concept preferred
