Job Summary
Hospice Social Worker (MSW) provides emotional, social, financial, and resource-based support to patients and families navigating serious illness and end-of-life care as a vital member of the interdisciplinary team. Roles include social casework, assessing family financial situations for potential assistance, coordinating referrals and services, participating in IDG team conferences, referring to community resources, and ensuring use of the 4Ms (What Matters, Medications, Mentation, Mobility) and Age-Friendly Care. Requires a Master’s in Social Work, current state social work license, valid driver’s license, strong clinical judgment, excellent communication, and proficiency with EMR and standard software. Coverage area includes Ann Arbor, Okemos, and Flint areas.
Required Qualifications
- Master's degree in Social Work (MSW) from CSWE-accredited program
- Current Social Worker License in state of practice
- Valid driver's license and good driving record
- Proficiency in clinical skills
- Availability during agency operating hours or as needed
- Ability to make sound professional clinical judgments
- Ability to assess and document patient needs and formulate individualized care plans
- Excellent verbal and written communication skills
- Proficiency with computer use including email, EMR, word processing, and spreadsheets
