Job Summary
Assess psychosocial needs, evaluate family dynamics, and provide counseling or crisis intervention to patients and caregivers navigating terminal illness. Guide end-of-life planning by educating families on advance directives, assisting with financial and legal resources, and connecting them to community agencies. Collaborate with physicians, nurses, and chaplains to develop individualized care plans, contribute psychosocial insights to interdisciplinary meetings, and advocate for patient dignity throughout the hospice journey. Maintain accurate clinical documentation, communicate changes in needs to the team, and uphold professional ethical standards. Requires a Master's degree in Social Work with one year of hospice experience, valid driver's license, and reliable transportation for home visits.
Required Qualifications
- Master's degree in Social Work (MSW)
- Meets applicable hospice personnel qualifications and state requirements
- 1+ year of MSW experience in a hospice setting
- Understanding of hospice philosophy and issues related to death, dying, grief, and end-of-life care
- Valid driver's license, reliable transportation, and automobile insurance if making home visits
