Job Summary
As a part-time Social Worker MSW in Home Health, you will assist patients and families in understanding and applying medical recommendations, develop individualized Plans of Care, locate and refer to community resources, and coordinate with the primary nurse on patient needs and discharge planning. You will maintain thorough documentation, serve as a resource for nursing staff regarding community supports, advocate for patients, conduct home visits to assess social, emotional, and financial factors, participate in staff meetings and case conferences, and engage in community outreach to connect patients with services. Strong English communication, a valid Florida driver’s license, and a graduate social work education are required; BLS/CPR is preferred. The position covers Lake County with a schedule of Monday–Friday, 8am–5pm, 3 days per week at AdventHealth Waterman in Tavares, FL.
Required Qualifications
- Master's degree in Social Work (MSW)
- Valid Florida Driver's License
- Fluent in English
- Graduate of an approved social work program
- Driver's License (DL) required
- CPR/BLS certification preferred
