Coordinate care management and behavioral health interventions for patients with complex needs by assessing, planning, and providing therapeutic interventions. Collaborate with physicians, families, and community agencies to facilitate lifestyle changes, manage crisis situations, and advocate for patient desires across the continuum of care. Participate in care conferences and identify appropriate levels of care to ensure cost-effective, quality outcomes. Requires an MSW degree, Michigan driver's license, and adherence to a 40-hour day shift schedule at the Continuing Care site in Grand Rapids.
