Job Summary
Conduct psychosocial assessments, develop care plans, and provide counseling and crisis intervention for patients and families. Identify decision-makers for patients without capacity, educate on rights, and facilitate referrals to hospital resources and community agencies including medication assistance programs. Collaborate with interdisciplinary healthcare teams to coordinate placement with home health agencies, long-term acute hospitals, skilled nursing facilities, and assisted living facilities. Serve as a resource for emotional and behavioral issues, referring clients to community services for adolescents, adults, and elders. Requires an MSW from a CSE-accredited program with three years of clinical social work or allied health practice experience. Located at St. Luke's Health–Brazosport Hospital in Lake Jackson, Texas.
Required Qualifications
- Masters Other from a Council on Social Work Education accredited graduate school of social work
- Three (3) years clinical social work
- Master Social Worker: TX
