Job Summary
Social Worker (MSW) focused on care management and interdisciplinary collaboration. Responsibilities include responding to referrals from the interdisciplinary team, conducting psychosocial assessments, developing and implementing care plans to ensure continuity of care, facilitating transitions to the next level of care, providing education and consultation, and serving as a liaison to the healthcare team addressing patients' and families' emotional, psychological, social, financial, and developmental aspects of care.
Required Qualifications
- MSW preferred
- Master's Degree in Social Work or related field
