Job Summary
Per Diem Social Worker with an MSW providing psychosocial assessments for dialysis, mother baby and oncology patient populations; collaborates with the interdisciplinary team in discharge planning for high-risk patients; participates in interdisciplinary rounds; educates patients and caregivers with appropriate community resources to help ensure safety upon discharge; functions under NASW guidelines with ongoing professional growth; requires NJ licensure (LSW) and MSW from an accredited program; strong writing and communication skills and ability to function well as part of a team; physical requirements include lifting 20-50lbs and various movement tasks; benefits information available.
Required Qualifications
- Graduate from an accredited School of Social Work with an MSW (Masters in Social Work) degree
- State licensure required by the NJ State board of social work examiners
- Licensed social worker (LSW) or licensed clinical social worker (LCSW) depending upon years of social work experience and clinical courses
- Minimum of one year of post-master9s in social work in the healthcare setting preferred
- Current licensure as a Social Worker in New Jersey, as an LSW required
