Job Summary
Provide psychosocial assessments and therapeutic support as part of an interdisciplinary healthcare team to assist patients and families with complex psychosocial needs. Responsibilities include alternate site coordination, discharge planning, addressing abuse/neglect, adoption, substance use disorders, end-of-life decisions, providing referral resources, bereavement support, adjustment counseling, facilitating guardianships and behavioral health concerns, and documenting assessments and plans in the EMR. Communicate with alternate care sites and community resources, participate in stakeholder committees as assigned, stay current with federal/state regulations, and support care transitions across settings. This role requires a Master’s in Social Work, 2 years of health care experience, and the ability to work in a hospital environment in Big Rapids, MI.
Required Qualifications
- Master's Degree in Social Work (MSW)
- Minimum 2 years of health care related experience (post-Master's)
- MSW licensure in Michigan upon hire (or ability to obtain)
- Preferred: Care management experience
- Preferred: Experience with individual/family assessment, crisis intervention, grief and loss counseling, and discharge planning
