Job Summary
Conduct comprehensive psychosocial assessments, develop person-centered care plans, and coordinate discharge planning for patients/residents and their families. Provide therapeutic interventions, facilitate groups, and advocate for the rights, interests, and well-being of individuals while supporting them through transitions. Assist with planning and implementing a comprehensive Social Services program ensuring compliance with federal and state regulations. Educate patients, families, and staff on health care decision making, advance directives, and available community resources. Requires a Master's degree in Social Work or Human Services, state licensure for long-term care practice, and 1-3 years of supervised healthcare experience. Additional certifications in Geriatric Case Management, Hospice & Palliative Care, or Mental Health are preferred. Required Qualifications
- Masters degree in Social Work or Human Services field
- any certifications/licensures as required by State of employment to practice in long-term care
Desired Qualifications
- 1-3 years of supervised social work experience in healthcare setting working directly with individuals
- Additional certification such as Geriatric Case Management, Hospice & Palliative Care, Gerontology, Clinical Social Work, Health Care, Nephrology, Mental Health, and/or Substance Use
